Step 1: Review Your Intake Forms on Jane App

You’ll receive a link from our Jane system that will prompt you to complete some forms. You will be asked to complete at least two forms, plus an additional form if you’re here for relationship counselling.

Please complete your forms at least 48 hours prior to your first session to ensure that your therapist can focus on gathering more specific information in your first session!

For your convenience, we’ve linked all the forms here so you can have a quick glance at them or save them for your personal reference, but you’ll receive fillable copies through Jane App.

 
 

This is the only form that is required to be reviewed and signed before your first session! The Informed Consent Form reviews all of our policies and your rights and responsibilities as a client. We can’t proceed through therapy without it, so please take a moment to review it and jot down and questions you may have!


If you are working with one of our art therapists, even if you’re not planning on doing art therapy, they have a slightly different Informed Consent form to sign – since they are trained in this type of work, we have some additional information to include that you should know about.


The intake is a starting point to help guide your therapist in building an understanding of what is going on for you – your first session (and generally the first few!) are meant to help gather information in more depth and will build upon and deepen the questions in the form. While this information is helpful, it is not required. We leave it to you to decide if you are comfortable sharing this information with your therapist before meeting them. You may also submit this at a later date if preferred. 


This is an additional form we ask couples to complete to give us some additional background and history to your relationship!


 If you would prefer not to complete the intake form, please ensure that the Informed Consent Form is filled out as this is the only form that is required to have on file to begin sessions.

 Your counsellor will review the Informed Consent form with you in your first session as well.
